Emergency services converged on the scene and police cordoned off the site as the helicopter navigated overhead cables, lamp posts and trees to make a rare landing on the busy road.
One of the victims was airlifted to Selly Oak Hospital with suspected head and back injuries while the other four were treated at Good Hope.
Part of the road was closed and traffic diverted when the incident happened at 12.35pm on Wednesday afternoon, near to the junction with Cremorne Road.
Police confirmed the car, a BMW Mini, had been in collision with a Mitsubishi L200.
Three land ambulances, police and the fire service were also in attendance.
A West Midlands Ambulance Service spokesperson said: "On arrival at the scene, crews found a car that had been in collision with a house.
"It is believed the car had been in collision with another vehicle prior to leaving the road.
"Both the car and the property had suffered substantial damage.
"Five teenage boys, believed to be approximately 18 years of age, were travelling in the vehicle at the time of the collision.
"The occupants were trapped in the car for a period of time.
"Once freed, they were all assessed and treated on scene.
"Four of the occupants, including the driver, were treated for back and minor injuries before being immobilised and conveyed to Good Hope Hospital for further assessment and treatment.
"A fifth occupant was treated for a suspected head and back injury before being airlifted to Selly Oak Hospital.
"No one else was injured in this incident."
Traffic heading towards Mere Green was diverted down Little Sutton Lane and vehicles travelling in the opposite direction were diverted into St James Road.
It is the third time in as many weeks the Air Ambulance has been called to attend emergencies in the area.
